The Board of Directors of the Florida AGD has re-established the
ANNUAL MEMBERSHIP AWARDS COMMITTEE as a standing committee of the FLAGD. The Committee is charged with establishing and selecting the
winners of a new series of awards for the following purposes:
1. To show appreciation by rewarding deserving individuals for
their achievements in the dental profession and in the Florida AGD.
2. To help encourage interest in the FLAGD among our general
membership.
3. To reward FLAGD Members for their accomplishments in earning
Continuing Education Credits.
4. To recognize the accomplishments of deserving Senior Dental
Students at each of the Universities in Florida.
The
Awards Committee has established certain achievement and merit
awards listed below. The achievement awards will recognize, when
appropriate, members of the Florida AGD who have made a significant
impact on our profession.
Because of their unique nature, not all achievement awards will be
presented each year. They will be presented only when the Committee
feels that a given nominee deserves the award.
ACHIEVEMENT AWARDS:
The J. Frank Collins
Lifetime Achievement Award
This award will be presented to an individuals who has
demonstrated innovation and leadership, dedication to the
advancement of dentistry, tenacity in pursuing idealistic goals, and
who has made positive changes for the benefit of the Florida AGD,
the dental profession and the dental health of the public.
The Distinguished Service Award
This award will be presented to a council, committee, or task force
member who has served the Florida AGD in an outstanding manner. The
recipient will be someone who has gone beyond the call of duty in
assuring continuity and integrity in the operations of this
organization, as in the development of new programs, and has served
the changing needs of the membership.
Humanitarian Award
The Humanitarian Award will recognize an individual who volunteers
his or her service to an underserved population, and who shows a
consistent commitment to enhancing the image of general dentistry
through the continuous delivery of quality dental care and through
consistent leadership in areas of civic involvement.

Qualifications:
a. Each award is for CE hours attended during one
calendar year.
b. Each application must be confirmed
by the submission of a copy
of the nominees AGD Transcript.
Further, the application must be accompanies by all associated
documentation of course attendance.
SENIOR DENTAL STUDENT AWARDS:
A. There may be one or two award
recipients at each
dental
school within the State of Florida. The National AGD sets
forth the requirements for award consideration.
RECIPIENT DETERMINATION:
If sufficient nominations are received, the committee may present
the top three (3) nominees to the FLAGD Board for determination
during the Board's spring meeting. The award recipients will
be announced in the spring issue of the Florida Focus.
